Job Summary:
We are seeking a skilled and compassionate Clinical Care Navigator to join our team at Overland Park Regional Medical Center. As a key member of our cardiology department, you will play a vital role in coordinating the delivery of high-quality clinical care to our patients with complex arrhythmia and ablation needs.
Responsibilities:
- Build relationships with patients and physicians to coordinate a plan of care, including appointments, transportation, education, and support services.
- Represent the patient and family within the multidisciplinary environment, ensuring seamless communication and collaboration with internal and external care providers.
- Conduct internal and external outreach and marketing to promote our program and services.
- Perform phone follow-up within 72 hours of discharge to assess patient needs, reinforce discharge instructions, and evaluate perception of care.
- Work within facility clinical practice guidelines, policies, and procedures, and follow recommendations and guidelines set forth by the program's medical directors.
- Participate in the Performance Improvement program to develop and comply with treatment guidelines.
Requirements:
- Graduate of an accredited school of nursing required.
- A minimum of three years of experience in specialty area of cardiology with demonstrated clinical competence.
