Job Summary

We are seeking a highly skilled and compassionate EP RN Navigator to join our team at Overland Park Regional Medical Center. As a key member of our healthcare team, you will play a vital role in coordinating the delivery of high-quality clinical care to our patients.

Key Responsibilities
  • Build relationships with patients and physicians to coordinate a plan of care, including appointments, transportation, education, and support services.
  • Act as a liaison between patients, families, primary care physicians, and internal and external care providers to ensure seamless care coordination.
  • Conduct phone follow-ups within 72 hours of discharge to assess patient needs, reinforce discharge instructions, and evaluate perception of care.
  • Work within facility clinical practice guidelines, policies, and procedures to ensure compliance and quality care.
  • Participate in Performance Improvement programs to develop and implement treatment guidelines and improve patient outcomes.
Requirements
  • Graduate of an accredited school of nursing required; BSN preferred.
  • Current licensure as a registered professional nurse in the state(s) of practice and/or active compact license.
  • Ability to achieve and maintain BCLS certification; ACLS certification required within 6 months of hire.
  • Minimum of three years of experience in specialty area of cardiology with demonstrated clinical competence.
