Job Summary:

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at the City of Portsmouth, VA. Responsibilities:

  • Provide medical services and medication-related activities to clients, including ordering supplies, documenting records, and performing medical assessments.
  • Administer injections, check vital signs and weight, screen for medication side effects, and interact with pharmacies and primary care agencies.
  • Provide wellness education classes and work closely with Central Intake staff to perform requirements of STEP VA.
  • Document progress and follow-up daily, and meet with clients and clinical staff to coordinate medical compliance.
  • Coordinate with aftercare pharmacy to order medications, enter medication information into the computer, and receive, store, and deliver psychotropic medications in a timely manner.
  • Communicate with pharmacies, consumers, providers, and others regarding requests for medication information and refills.
  • Perform psychotropic medication injections as ordered by physician and respond to physician requests for medication samples.
  • Screen consumers to qualify for pharmacy assistance programs, perform and record consumers' vital signs and weight under the order of physician, and obtain signed release of information and requests records from outside healthcare providers for consumer medical information.
  • Monitor and order various medical supplies, including prescription pads, needles, and syringes, and maintain first aid supplies and accounting system for supplies.
  • Maintain documentation and monitoring guidelines, keep abreast of changes in the medication ordering system requirements, and communicate to consumers as indicated.

Requirements:

  • Certification as a Licensed Practical Nurse (LPN) in the State of Virginia or license eligible.
  • 1-2 years of progressively responsible experience providing nursing services in the behavioral and/or mental illness field.
  • CPR certification.
  • An acceptable general background check, including a local, state, and criminal history and sex offender registry check.
  • A valid driver's license with an acceptable driving record.
