We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at the City of Portsmouth, VA. As a LPN, you will play a vital role in providing medical services and medication-related activities to our clients.

Job Summary:

The LPN will be responsible for:

  • Providing medical services and medication-related activities to clients
  • Ordering supplies and documenting records
  • Performing medical assessments on clients at intake and making referrals as needed
  • Administering injections, checking vital signs and weight, and screening for medication side effects
  • Interacting with pharmacies and primary care agencies
  • Providing wellness education classes and working closely with Central Intake staff to perform requirements of STEP VA
  • Documenting progress and following up daily with clients and clinical staff to coordinate medical compliance
Requirements:

To be successful in this role, you will need:

  • Certification as a Licensed Practical Nurse (LPN)
  • 2 years of progressively responsible experience providing nursing services in the behavioral and/or mental illness field
  • CPR certification
  • An acceptable general background check, including a local, state, and criminal history and sex offender registry check
  • A valid driver's license with an acceptable driving record
