A. POSITION SUMMARY

Position Title: Social Service Coordinator HCP123

Position Number:

Work Unit: AACSB Pataula Treatment Court

Immediate Supervisor's Title: Western Co. Coordinator

Supervisory Responsibilities: Oversees SSP 1 and SST positions.

B. REQUIRED QUALIFICATIONS:

The ideal candidate will possess the ability to conduct comprehensive needs assessments for program services and evaluate the requirements of the client population. Familiarity with agency and federal policies, procedures, and guidelines is essential. The candidate should have a solid understanding of diagnostic criteria and client advocacy systems, along with crisis intervention techniques. Strong communication skills are necessary to effectively engage with diverse populations.

Additional qualifications include:

  • Knowledge of individual and group therapy methodologies.
  • Understanding of various therapeutic models and interventions relevant to the assigned area.
  • Awareness of medication types and their effects.
  • Expertise in behavioral standards and clinical skills.
  • Capability to develop and implement treatment or behavioral plans to address crises and deliver services.
  • Ability to establish and prioritize program goals and objectives.
  • Experience in guiding policy and procedure development.
  • Oversight of compliance with all applicable regulations.
  • Familiarity with DSM-IV criteria and treatment approaches.
  • Skill in completing and interpreting psychosocial assessments.
  • Knowledge of family dynamics and support groups.
  • Ability to facilitate family support initiatives.
  • Understanding of confidentiality policies and the ability to identify and address potential obstacles for clients.
  • Competence in conveying behavioral change concepts through effective teaching methods.
  • Ability to maintain accurate case files in accordance with established policies.
  • Demonstrated culturally sensitive interviewing techniques.
  • Capability to coordinate services across various community resources.
  • Knowledge of program-related regulatory agencies and the audit process.
  • Experience in performance management and employee evaluations.

Entry qualifications include a Master's Degree with licensure and three years of relevant postgraduate experience, including at least one year in a supervisory capacity, or a Master's Degree with three years of experience at a lower level or equivalent position.

C. EQUIPMENT USAGE:

Includes emergency and treatment equipment requiring safe and effective use.

D. BLOODBORNE PATHOGEN CATEGORY:

Employee performs assigned tasks with no exposure to blood or other potentially infectious materials.

E. TRAINING REQUIREMENTS:

Direct Care training is mandatory.

G. KEY RESPONSIBILITIES:

The Social Service Coordinator provides essential psychosocial support to individuals, families, or vulnerable populations facing chronic, acute, or terminal conditions, as well as physical or developmental disabilities. Responsibilities include:

  • Acting as an advocate for clients to coordinate necessary services and resolve crises.
  • Administering and coordinating specialized program services and activities.
  • Conducting individual and group counseling sessions to assist clients in overcoming dependencies and adjusting to life changes.
  • Monitoring, evaluating, and documenting client progress in alignment with measurable goals outlined in treatment care plans.
  • Referring clients or families to community resources to facilitate recovery from mental or physical health challenges.
