The Registered Nurse, RN, is a vital member of the interdisciplinary team at VITAS Healthcare, dedicated to providing exceptional end-of-life care to patients and their families. As a skilled nurse, you will be responsible for coordinating patient care, ensuring continuous assessment of each patient's needs, and implementing the interdisciplinary plan of care.

Key Responsibilities:

  • Case Management and Supervision: Provide coordination of care, ensure continuous assessment of each patient's needs, and implement the interdisciplinary plan of care.
  • Nursing Care: Provide primary nursing care for inpatient and continuous care patients, including physical, psychosocial, spiritual, and bereavement assessments.
  • Quality Improvement: Participate in the Quality Assessment and Performance Improvement (QAPI) Program and related activities, including Performance Improvement Projects (PIPs) and gathering clinical data for quality measures.
  • Professional Development: Develop and achieve professional growth goals and objectives, attend orientation and regularly scheduled in-services/educational conferences, and participate in training activities internally and externally.

Requirements:

  • Current licensure to practice nursing in the state where the VITAS program is located.
  • A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
