Join Brooke Grove Foundation as a Licensed Practical Nurse (LPN) in our Assisted Living community. We are committed to providing high-quality care and services that enrich the lives of our residents and employees. As an LPN, you will play a vital role in delivering exceptional care and support to our residents. Your responsibilities will include performing resident assessments, visiting residents on a routine basis, and completing all clinical documentation in accordance with regulatory requirements. You will work collaboratively with nursing administration and healthcare team members to ensure follow-through of resident care and care plans. Additionally, you will coordinate and monitor duties performed by geriatric nursing assistants, provide coaching and verbal/written disciplinary action as necessary, and monitor medication administration performed by certified medicine aides. You will also contact physicians, specialists, and other care providers as needed to ensure appropriate communication and follow-through of resident care. As a member of our team, you will have the opportunity to make a positive impact on the lives of our residents and employees.
