Job Title: RN Case Manager

St. Charles Health System is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring the delivery of effective and efficient patient care.

Job Summary:

The RN Case Manager will provide clinically based case management to support the delivery of effective and efficient patient care. This position has the overall accountability for the utilization management and discharge planning for patients within the assigned caseload.

Key Responsibilities:
  • Utilize the nursing process of assessment, diagnosis, planning, intervention, and evaluation when assessing the patient's condition and needs.
  • Identify and confirm medical necessity for continued stay.
  • Create a discharge plan appropriate to the patient's needs and resources.
  • Collaborate with team members to facilitate patient's and family's learning throughout the hospital experience in preparation for discharge.
  • Reinforce patient's continued health care through teaching and/or referral to community agency follow-up.
  • Document in the patient record according to SCHC policies and procedures.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current Oregon RN License.
  • AHA Basic Life Support for Healthcare Provider certification.
Preferred Qualifications:
  • Certification in Case Management.
  • Experience in case management and/or discharge planning.
Work Environment:

This position requires the ability to work in a fast-paced environment and to prioritize tasks effectively. The RN Case Manager will work closely with the healthcare team to ensure the delivery of high-quality patient care.
