Job Summary:

The Registered Nurse (RN) provides nursing care to terminally ill Hospice patients as needed. This role requires a strong understanding of Hospice care and the ability to work independently, making accurate and sometimes quick judgments.

Responsibilities:

  • Assess and reassess the nursing needs of Hospice patients
  • Provide dietary counseling and Hospice nursing services, treatments, and preventive procedures
  • Initiate nursing procedures appropriate for the patient's Hospice care and safety
  • Observe signs and symptoms and report to the physician and IDG members any unexpected changes in the patient's physical or emotional condition
  • Teach, supervise, and counsel Hospice patients and family members about providing care for the patient
  • Supervise and train other nursing service personnel
  • Develop and re-evaluate the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care
  • Perform specific nursing procedures as needed, following doctor's orders
  • Attend team conferences and maintain records as required by Hospice
  • Follow the policies and procedures of Hospice, observing confidentiality and safeguarding all patient-related information in compliance with HIPAA regulations
  • Participate in on-call system and provide on-call coverage when unavailable for assigned duties
  • Work with the interdisciplinary group concept of patient care and coordinate the implementation of the plan of care for patients residing in SNF, NF, ICF, or MR
  • Organize work schedule and utilize time management to attend all required meetings
  • Comply with agency infection control policies and protocols
  • Assist with orientation, teaching, and training as requested

Requirements:

  • Graduate from an accredited school of nursing
  • Currently licensed as a registered nurse to practice in the state
  • Minimum 2 years of experience as a registered nurse, preferred
  • Active patient contact within the past three years, preferred
  • Ability to work independently, make accurate, and at times, quick judgments
  • Ability to supervise others appropriately
  • Ability to respond appropriately to crisis outside of a hospital setting
  • Acceptance of and adaptability to different social, racial, cultural, and religious modes
  • Must complete Hospice training program
