Job Title: Home Care Intake Coordinator RN

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team as a Home Care Intake Coordinator. This role is a critical part of our home health care team, responsible for assessing patients' needs and coordinating referrals to home care and hospice services.

Responsibilities:
  • Assess and evaluate patients' needs, developing an appropriate plan of care
  • Provide education and strategies for patients and families
  • Coordinate referrals to home care to promote continuity of care
  • Partner with other VNA clinicians and external providers to execute best patient outcomes
  • Document all changes in patients' conditions and update care plans and outcomes
Qualifications:
  • Current NJ RN licensure
  • 2+ years of home health nursing experience
  • Excellent verbal and written communication skills
  • Excellent interpersonal and organizational skills
  • Strong client focus, assertive, decision-making skills, articulate, and creative in problem-solving
Working Conditions/Physical Demand:

This role requires a fast-paced healthcare environment with phone, scanner, email, fine motor hand movement, and computer use. Use of a personal automobile with valid insurance and a driver's license is also required.
