Long Term Care Patient Care Assistant Opportunity

Hilltop Skilled Nursing and Rehabilitation is seeking dedicated Patient Care Assistants to become part of our expanding team.

As a Patient Care Assistant at our facility, you will engage in various responsibilities that ensure exceptional care for our residents. The role involves delivering routine daily nursing services to assigned residents in a manner that is compassionate, respectful, and aligned with regulations and facility standards.

Your Responsibilities:

  • Provide care and support to residents and their belongings with dignity and respect.
  • Assist with meal delivery and feeding as needed, ensuring residents stay hydrated.
  • Conduct regular rounds to ensure proper care and service for residents.
  • Assist in repositioning patients in bed to prevent discomfort and bedsores.
  • Monitor vital signs and patient behavior, reporting any changes to nursing and medical staff.
  • Support residents during bathing or showering activities.
  • Ensure proper skin care and promptly report any observations of skin issues.
  • Maintain reliable transportation and punctuality for assigned shifts.
  • Perform additional tasks as directed by management.

Who You Are:

  • Currently certified as a Nursing Assistant in the state of Illinois.
  • A reliable team player committed to maintaining a dependable work history.

  • A graduate of an accredited Certified Nursing Aide program.
