Position Overview:

As a Hospice Certified Nursing Assistant (CNA) with Continuum Hospice, you will be instrumental in the lives of our patients, collaborating with a registered nurse to deliver essential services that ensure their comfort and well-being.

Key Responsibilities:

Deliver Personalized Care:
Implement the carefully designed care plan to ensure the comfort and well-being of our patients. Respond promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.

Assist with Personal Care Tasks:
Aid patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments significantly contributes to their overall comfort and dignity.

Commit to Customer Service Excellence:
Prioritize exceptional customer service, striving to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be recognized and appreciated as part of our team.

Support Families:

Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones.

Your presence offers confidence and reassurance during this challenging time.

Maintain Accurate Records:

Every detail is vital in delivering high-quality care, both in the patient's home and within our agency and regulatory standards.

Qualifications:
High School Diploma or G.E.D. required.

Current unencumbered certification in the state as a C.N.A.

Minimum one (1) year experience as a C.N.A. in an acute care or long-term care setting. Hospice experience is a plus.

Possess and maintain current CPR certification if state-mandated.

$24.50 per hour (Average Pay Range). Pay will be determined based on experience.
