Brookhaven Hospice is seeking a dedicated Full-Time Registered Nurse Case Manager (RNCM) who is passionate about making a significant impact in the lives of patients and their families.
Work Schedule:
Monday to Friday, 8:00 AM - 4:30 PM, with occasional weekend and vacation rotations.
We are in search of a dynamic, empathetic, and detail-oriented Registered Nurse (RN) to serve as a Hospice Case Manager for our patients.
Our Hospice RN Case Managers are responsible for planning, organizing, and directing hospice care through the nursing process, which includes assessment, planning, interventions, implementation, and evaluation. They effectively engage with patients, caregivers, families, and other interdisciplinary team members while upholding the highest standards of professional nursing and clinical excellence.

Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate approach that reflects our commitment to exceptional service.

Work closely with our Clinical Director to evaluate the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in delivering the right care at the right time.
Direct Patient Care:

Qualifications:
Must hold a current unencumbered license as a Registered Nurse.
Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting; hospice experience is a plus.
Proficiency in computer skills, including typing and familiarity with various programs, particularly EMR.
Must possess and maintain current CPR certification.
