Nurse Case Manager

Tenet Massachusetts is seeking a skilled Registered Nurse to fill a Case Management position at one of our esteemed hospitals in Framingham, Massachusetts.
Job Description & Requirements- Specialty: Case Management

- Shift: 8 hours, days
- Employment Type: Staff
The successful candidate will be responsible for facilitating care along a continuum, ensuring effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. This role requires a strong understanding of medical necessity and the ability to assess patients for transition needs to promote timely throughput, safe discharge, and prevent avoidable readmissions.
The RN Case Manager will integrate national standards for case management scope of services, including utilization management, transition management, and care coordination. This role also requires compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy.
Onboarding Process: Please note that candidates must successfully complete a background check and pre-employment health screening, which includes a drug screen.
