Job Summary:The RN Case Manager will be responsible for facilitating care along a continuum, ensuring that patients receive the appropriate level of care based on medical necessity. This role requires strong communication and coordination skills, as well as the ability to assess patients for transition needs and promote timely throughput, safe discharge, and prevent avoidable readmissions.
Responsibilities:- Utilization Management: Support medical necessity and denial prevention
- Transition Management: Promote appropriate length of stay, readmission prevention, and patient satisfaction
- Care Coordination: Demonstrate throughput efficiency while ensuring care is the right sequence and at the appropriate level of care
- Compliance: Ensure compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy
- Education: Graduate of an accredited School of Nursing; BSN preferred
- Certificates, Licenses, Registrations: Current RN licensure in State of Massachusetts
- Knowledge and Skills: 2 years RN acute care hospital experience; acute care case management experience preferred; Accredited Case Manager (ACM) certification preferred
