Senior Healthcare Professional

We are seeking a dedicated and experienced Senior Healthcare Professional to join our team as a Home Health Care Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Estimated Salary Range: $60,000 - $80,000 per year
Responsibilities
- Provide high-quality patient care in the comfort of their own homes, focusing on health promotion, disease prevention, and management.
- Conduct thorough assessments, develop personalized care plans, and implement evidence-based interventions to achieve optimal patient outcomes.
- Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure seamless care coordination.
- Communicate effectively with patients, families, and caregivers to ensure informed decision-making and empowerment.
- Maintain accurate and timely documentation, adhering to regulatory requirements and clinical standards.
Requirements
- Minimum 1 year of experience in home health care or a related field, with a focus on gerontology or pediatrics.
- Demonstrated expertise in assessing patient needs, developing care plans, and implementing interventions to achieve optimal outcomes.
- Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a fast-paced environment.
- Current licensure, certification, and CPR/BLS training, as required by state and federal regulations.
