At MultiCare Health System, Inc., we are seeking a skilled Hospital Social Worker to join our team. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions of care for our patients.

The ideal candidate will possess a Master's degree in Social Work and a minimum of three years of experience in a hospital or equivalent setting. Strong communication and interpersonal skills are essential for success in this role.

Responsibilities will include:

  • Providing cross continuum care activities, including clinic, provider practice, urgent care, phone care agency, skilled care, or other sites of care, as appropriate
  • Offering customer service to patients, families, physicians, and care team members, including discharge milestones and progression to next level of care
  • Modeling and orchestrating consistent, professional communications and maintaining consistent and proactive communication patterns with colleagues and staff
  • Ensuring that patients and families experience medically appropriate transitions of care
