Job Summary

The Palliative Care Nurse Practitioner at Elite Home Health & Hospice plays a vital role in delivering high-quality patient care. As a key member of our team, you will be responsible for providing expert clinical services to patients in their homes, hospitals, and other healthcare settings. Your primary focus will be on alleviating the suffering of patients with serious illnesses and improving their quality of life.

Key Responsibilities

1. Provide comprehensive patient assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless care delivery.

2. Conduct thorough medical histories, perform physical examinations, and order diagnostic tests as needed to inform treatment decisions.

3. Prescribe medications, treatments, and therapies to manage patients' symptoms and improve their functional status.

4. Educate patients and their families on disease management, treatment options, and self-care techniques to promote independence and empowerment.

5. Collaborate with healthcare providers, social workers, and other stakeholders to ensure comprehensive care and address patients' psychosocial needs.

6. Maintain accurate and timely documentation of patient encounters, including progress notes, medication lists, and treatment plans.

7. Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.

8. Stay up-to-date with the latest clinical guidelines, research, and best practices in palliative care to provide evidence-based care.

Requirements

To be successful in this role, you will need to have a strong foundation in nursing practice, excellent communication and interpersonal skills, and the ability to work independently and collaboratively as part of a multidisciplinary team. A minimum of two years of experience in home health or palliative care is preferred, along with a current CPR certification and a valid driver's license. You will also need to be self-directed, able to work with minimal supervision, and possess excellent observation, problem-solving, and verbal and written communication skills.
