Join Our Team as an LPN MDS Coordinator

We are seeking a skilled LPN MDS Coordinator to join our team at the Lebanon Center in Lebanon, NH. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality of care for our residents.

About the Role

The LPN MDS Coordinator will be responsible for directing the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, following guidelines for completion, computer encoding, and electronic State transmission of the Minimum Data Set (MDS). This role requires a strong understanding of general, rehabilitative, and restorative nursing and medical practices and procedures, as well as laws, regulations, and guidelines governing long-term care.

Responsibilities
  • Direct the nursing center's interdisciplinary RAI process
  • Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes
  • Train new members of the interdisciplinary team on clinical content and electronic completion of the applicable MDS items
Requirements
  • Must be an LPN in good standing and currently licensed by the state
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility is required
  • Must have computer skills to include Windows applications
  • AccuMed experience is preferred
