Job Summary

We are seeking a highly motivated and compassionate Peer Navigator to join our team at Healthcare in Action. As a Peer Navigator, you will play a critical role in supporting individuals experiencing homelessness in the San Diego area. Your primary responsibility will be to provide peer support, education, and navigation services to help our members manage their health and achieve stability in their lives.

Key Responsibilities
  • Build strong relationships with members by sharing your personal experiences and providing a supportive and non-judgmental space for them to open up.
  • Develop and implement personalized strategies to help members manage their health conditions and behaviors, and connect them with local resources and services to address barriers to care.
  • Conduct regular outreach to members at the Navigation Center and in the community, providing support and advocacy as needed.
  • Accompany members to medical, dental, and other appointments, and provide transportation as necessary.
  • Collaborate with our team and community partners to ensure seamless care coordination and support for our members.
  • Document all interactions and progress in our electronic health records system, and complete required documentation in a timely and accurate manner.
Requirements
  • High school diploma or equivalent required; bachelor's degree preferred.
  • At least 3 years of experience working with individuals experiencing homelessness, or a comparable combination of education and experience.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Reliable transportation and a valid driver's license required.
  • Ability to work in a dynamic, outdoor environment and adapt to changing circumstances.
