About the RoleWe are seeking a skilled Behavioral Health Counselor to provide top-of-scope practice in direct patient care environments, delivering applicable behavioral health or mental health care. As a member of our multidisciplinary team, you will value a collaborative approach to achieve exceptional outcomes.
Key Responsibilities- Implement counseling/therapy for assigned patients in individual and group settings.
- Interview and assess patients, family members, caregivers, and legal representatives to determine needed services and interventions.
- Participate in case reviews and care conferences to evaluate patient progress.
- Assist with discharge planning, identifying community-based resources, and providing referrals as part of the discharge plan.
- Utilize teaching methods and referrals to ensure patient/family educational needs are met.
