Job Summary:

The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities.

This role is a vital member of our interdisciplinary team, providing expertise in psych-social assessments, interventions, and knowledge of community resources.

The RN will work in either a Case Management, Admissions, or Resource role, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.

Requirements:

  • Current Registered Nurse (RN) licensure in good standing
  • Excellent communication and teamwork skills
  • Ability to work independently and prioritize tasks effectively
  • Valid driver's license and reliable transportation
