Job Summary

We are seeking a highly motivated and compassionate Foster Care Placement Coordinator to join our team at the National Youth Advocate Program. As a key member of our team, you will be responsible for coordinating the placement of foster children with suitable families, ensuring the best possible outcomes for these vulnerable youth.

Key Responsibilities
  • Conduct initial assessments of referrals to determine the most suitable placement options for foster children.
  • Communicate effectively with referring agencies, foster parents, and other stakeholders to ensure seamless placement processes.
  • Recommend appropriate services based on assessment results, prioritizing the least restrictive environment for the child.
  • Enter referral information into agency systems, including SharePoint and Excel, to maintain accurate records.
  • Assist in increasing the referral base through approved methods and strategies, promoting NYAP's continuum of care and services.
  • Distribute statistical reports to supervisors detailing service reception activities as requested.
  • Advocate on behalf of the youth through individual and system advocacy to promote the provision of service delivery to the youth and their families.
Requirements
  • Minimum two years' experience in youth services, preferably with direct experience in Human Services.
  • A Bachelor's Degree in a social service or related field.
  • Proficient use of desktop and laptop computers, smartphones, and tablets, as well as software including word processing, spreadsheet, and database programs.
