- Serve as the point of contact for all disciplines involved in patient care, overseeing the frequency of visits and communicating with physicians and office staff as needed.
- Collect accurate OASIS data, maintain current medication profiles, and report lab values to physicians in a timely manner.
- Attend weekly case conferences and monthly case manager meetings to stay informed and collaborate with colleagues.
- Current licensure as a Registered Nurse in the state of employment.
- A minimum of one year of clinical experience preferred.
- Demonstrate knowledge and skill in current nursing practice.
