About the Role

As a Case Manager, you will be a vital member of an interdisciplinary team, working collaboratively to deliver exceptional patient care and support patients in their recovery journey.

Key Responsibilities

  • Develop and implement individualized care plans to achieve patient goals and outcomes
  • Collaborate with healthcare professionals to identify barriers to care and develop solutions
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making
  • Coordinate patient care team conferences to refine treatment plans and achieve desired outcomes
  • Review patient records to identify potential comorbid conditions and communicate findings to the HIM team

Requirements

  • Current licensure as a Registered Nurse, Social Worker, or Healthcare professional
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum of 2 years experience in an inpatient setting, preferably in acute/rehabilitation hospital
  • Effective communication skills in English, additional languages a plus
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
