End-of-Life Care Coordinator

We are seeking a compassionate and skilled Registered Nurse to join our team as an End-of-Life Care Coordinator. In this critical role, you will provide expert nursing care and coordination of services to patients and their families facing terminal illness and bereavement.
About the Role
The End-of-Life Care Coordinator will work collaboratively with the interdisciplinary team to develop, implement, and evaluate individualized plans of care for patients in various settings, including inpatient facilities and community-based programs. You will be responsible for assessing patient needs, coordinating services, and providing education and support to patients and their families. Your expertise in pain management, symptom control, and end-of-life care will be invaluable in ensuring that patients receive high-quality care and support during this difficult time.
Responsibilities
This is a full-time position that requires reliable transportation and adequate insurance coverage for driver and passengers. As a key member of our team, you will participate in ongoing training and professional development activities to enhance your skills and knowledge in end-of-life care. You will also contribute to quality improvement initiatives and participate in peer review and evaluation processes. Key Skills and Qualifications
To be successful in this role, you will need to possess the following skills and qualifications:
• Current licensure as a Registered Nurse in the state where the VITAS program is located
• A minimum of two years of experience in hospice, home health, or community health in the last five years
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
• Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments
• Knowledge of regulatory requirements and industry standards related to end-of-life care
Benefits
In addition to a competitive salary range of $62,000 - $90,000 per year, depending on experience, we offer a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) retirement plan, paid time off, and continuing education opportunities.
