Job Description Summary

The occupational therapist contracted through Freedom Home Health And Hospice Care Services Inc is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

Essential Job Functions/Responsibilities
  • Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity. Assess the home environment for hazards or barriers to independent living, identify equipment needs, and participate in the development of the total plan of care.
  • For patients planning to return to work, the occupational therapist may perform work capacity evaluations and refer to specialized vocational training in accordance with Freedom Home Health And Hospice Care Services Inc policy.
  • Develop treatment programs and establish goals for improved function. Communicate the plan of care to referring physicians and other healthcare professionals.
  • Teach new skills or retrain patients in daily activities lost due to illness or injury, in accordance with Freedom Home Health And Hospice Care Services Inc policy.
  • Fabricate splints and instruct patients in the use of adaptive equipment to improve function.
  • Train patients in the use of prosthetic and/or orthotic devices.
  • Maintain accurate clinical records, clinical notes, and reports to physicians any changes in patients' conditions. Submit these records in accordance with Freedom Home Health And Hospice Care Services Inc policy.
  • Maintain contact/communication with other personnel involved in patients' care to promote coordinated, efficient care.
  • Attend and contribute to in-services, case conferences, and other meetings as required by Freedom Home Health And Hospice Care Services Inc policy to ensure coordinated and comprehensive plans of care for patients.
  • Identify patient and family/caregiver needs for other home health services and refer as appropriate.
  • Instruct patients' family/caregivers and other healthcare personnel in patients' treatment regimes when indicated and appropriate.
  • Supervise certified occupational therapy assistants according to Freedom Home Health And Hospice Care Services Inc policy and state regulations.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
  • Participate in discharge planning for patients.

Position Qualifications
  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Two years of clinical experience as an occupational therapist.
  • Demonstrate good verbal and written communication, and organization skills.
  • Possess and maintain current CPR Certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or Freedom Home Health And Hospice Care Services Inc requirements and is in good working order.
