Position: RN Patient Care Navigator - Transplant

Inova Health System is seeking a dedicated RN Patient Care Navigator to enhance our Transplant Team in Heart Failure. This is a full-time position, Monday through Friday, with rotations between facilities.

Key Responsibilities:

Act as a liaison and advocate for patients, ensuring they receive optimal healthcare outcomes through effective communication with healthcare providers. Assist patients and their families in navigating financial, psychosocial, and administrative challenges by providing guidance and referrals to appropriate resources. Offer emotional support, clinical education, and expert advice to empower patients and families in their healthcare journey. Build collaborative relationships with patients, healthcare professionals, and community resources to enhance patient outcomes and address barriers to care. Continuously seek to improve the healthcare experience and facilitate seamless continuity of care. Support patients and families throughout the healthcare process, from diagnosis to follow-up care.

Qualifications:
  • Employment Type: Full-time, Monday to Friday
  • Location: Rotational assignments between facilities
  • Education: Bachelor of Science in Nursing (BSN)
  • Experience: Minimum of 3 years in a relevant nursing role
  • Licensure/Certifications: Registered Nurse License in Virginia; BLS certification from the American Heart Association
