Job SummaryWe are seeking an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Southern New Jersey region.
You will work under the supervision of a physician, applying medical techniques and principles to care for a diverse set of patients with chronic diseases.
Key Responsibilities- Provide high-quality care to primary and palliative patients in and around Monmouth County, NJ
- Assess patients' health by performing physical examinations, obtaining and updating medical histories, and creating personalized care plans
- Effectively communicate with office staff and clinical operations across various internal platforms
- Provide patient-centered care to a panel of patients, fostering trust and consulting with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary and prescribe medications, supplies, and interventions
We require a Board Certified Nurse Practitioner with a valid NP certification in the state of New Jersey, strong clinical skills, and the ability to accurately and confidently treat, assess, and diagnose.
A positive demeanor, strong people skills, and the ability to problem-solve and operate independently are essential.
1-2 years of primary care experience with geriatric patients, preferably with hospice experience, is highly preferred.
