As an Inpatient Pharmacist Specialist at Kaiser Permanente, you will play a critical role in ensuring the safe and effective use of medications for our patients. Under the supervision of a licensed pharmacist, you will monitor and manage the medication therapy of hospitalized patients, provide medication information and pharmacokinetic dosing advice, and adjust doses and orders laboratory tests as needed. You will also be responsible for dispensing, compounding, and distributing pharmacy products, as well as providing direction to ancillary pharmacy staff.
Essential Responsibilities:- Uphold Kaiser Permanente's policies and procedures, as well as applicable state and federal laws.
- Perform sterile and non-sterile compounding, and collaborate with the healthcare team to develop and implement patient-specific plans to optimize medication therapy.
- Monitor medication therapy to ensure optimum therapeutic results, and manage and optimize medication therapy based on interpretation of data, assessments, and evaluations of patient outcomes.
- Follow hospital-approved collaborative, pharmacokinetic drug therapy management protocols, and interview patients and family members or other designated caregivers to obtain information regarding medication use, medication allergies, and sensitivities.
- Document information appropriately in the patient's medical record or other computer systems, and advise patients and designated family members or other designated caregivers regarding medication, directions for use, storage requirements, medication coordination with diet, importance of compliance, precautions, and warnings for medication therapy.
- Provide pharmacokinetic dosing advice to medical personnel, and order and adjust medication dosages, order laboratory tests, and administer medication under protocol according to established policies and procedures.
- Monitor the effectiveness and accuracy of the hospital medication distribution system, and review all medication orders for appropriateness and adherence to the drug formulary.
- Clarify orders and intervene when necessary, and verify accuracy of medication preparation and dispensing by pharmacy personnel.
- Provide medical personnel with medication information, and identify potential medication safety issues and work actively to prevent medication errors.
- Review patient medical records to detect adverse drug events, and assist medical personnel to form treatment plans in response to adverse drug events.
- Evaluate adverse drug events to determine their severity, probability, and preventability for further analysis and review.
- Review patient medical records to verify proper delivery and correct administration of medication, and ensure appropriate medication storage and security.
- Participate in investigational drug studies as required, and participate in problem identification and resolution, consulting with supervisors when appropriate.
- Support a collaborative Labor-Management Partnership environment, and report discrepancies to appropriate medical and/or administrative personnel.
- Develop and test requirements and specifications for protocols, computer programs, pharmacokinetic dosing, and other pharmaceutical care applications.
- Participate in assigned education and training programs, and develop and maintain teaching curriculum and conduct educational programs for pharmacy, other medical personnel, and patients.
- Actively participate in rounds, hospital committees, utilization review, quality management programs, and other programs as needed.
- Participate actively as a member of the interdisciplinary team during medical emergencies, such as Code Blue.
- Possess a minimum of 1200 hours of inpatient pharmacy experience within the last 5 years, or successful completion of an ASHP-accredited clinical pharmacy practice residency.
- Degree in Pharmacy meeting minimum education requirements for licensure.
- Controlled Substance Utilization Review and Evaluation System Registration required at hire.
- Pharmacist License (California) required at hire.
- National Provider Identifier required at hire.
- Advanced Cardiac Life Support within 3 months of hire from American Heart Association.
- Basic Life Support within 3 months of hire from American Heart Association.
- Demonstrate knowledge of patient variables such as age, sex, and other physical characteristics/presentation and disease states as they relate to medication therapy.
- Comprehensive knowledge of acute care medications.
- Possess comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, and disease states, and the ability to relate this information to the patient's medical management plan.
- Demonstrated work experience performing aseptic technique and the ability to prepare sterile IV products and chemotherapeutic agents.
- Excellent verbal and written communication skills.
- Knowledge of Federal and State laws and regulations regarding the provision of pharmaceutical services.
- Passage of a standardized competency assessment OR current incumbent in job code 60002, 60007, 67130 or.
- Demonstrated ability to extract pertinent information from the patient, or other sources, and relate it to the patient's medical management plan.
- Experience operating pharmacy computer system(s) and other equipment.
- Comprehensive knowledge of emergency medications.
- Doctorate of Pharmacy, Pharm.D.
