Inpatient Pharmacist Specialist

As a skilled Inpatient Pharmacist Specialist, you will play a vital role in ensuring the safe and effective use of medications for hospitalized patients. Under the guidance of experienced medical professionals, you will monitor and manage medication therapy, provide medication information, and collaborate with the healthcare team to optimize patient outcomes. Your expertise will be essential in preventing medication errors, identifying potential safety issues, and developing patient-specific plans to optimize medication therapy.
Key Responsibilities:
- Uphold Kaiser Permanente's policies and procedures, as well as applicable state, federal, and local laws.
- Perform sterile and non-sterile compounding, ensuring the highest standards of quality and safety.
- Collaborate with the healthcare team to develop and implement patient-specific plans to optimize medication therapy.
- Monitor medication therapy to ensure optimal therapeutic results and identify potential safety issues.
- Manage and optimize medication therapy based on interpretation of data, assessments, and evaluations of patient outcomes.
- Follow hospital-approved collaborative, pharmacokinetic drug therapy management protocols.
- Interview patients and family members to obtain information regarding medication use, allergies, and sensitivities.
- Document patient information and medication management plans in the patient's medical record or other computer systems.
- Provide medication information and advice to patients and healthcare professionals.
- Order and adjust medication dosages, order laboratory tests, and administer medication under protocol according to established policies and procedures.
- Monitor the effectiveness and accuracy of the hospital medication distribution system.
- Review medication orders for appropriateness and adherence to the drug formulary.
- Clarify orders and intervene when necessary.
- Verify the accuracy of medication preparation and dispensing by pharmacy personnel.
- Provide medical personnel with medication information and support.
- Identify potential medication safety issues and work actively to prevent medication errors.
- Review patient medical records to detect adverse drug events and assist medical personnel in forming treatment plans.
- Evaluate adverse drug events to determine their severity, probability, and preventability.
- Review patient medical records to verify proper delivery and correct administration of medication.
- Ensure appropriate medication storage and security.
- Participate in investigational drug studies as required.
- Participate in problem identification and resolution, consulting with supervisors when necessary.
- Support a collaborative Labor-Management Partnership environment.
- Report discrepancies to appropriate medical and/or administrative personnel.
- Develop and test requirements and specifications for protocols, computer programs, and pharmacokinetic dosing applications.
- Participate in assigned education and training programs.
- Develop and maintain teaching curriculum and conduct educational programs for pharmacy, medical personnel, and patients.
- Actively participate in rounds, hospital committees, utilization review, quality management programs, and other programs as needed.
- Participate actively as a member of the interdisciplinary team during medical emergencies.
Experience
- Possess a minimum of 1200 hours of inpatient pharmacy experience within the last 5 years or successful completion of an ASHP-accredited clinical pharmacy practice residency.
- Degree in Pharmacy meeting minimum education requirements for licensure.
- Controlled Substance Utilization Review and Evaluation System Registration required at hire.
- Pharmacist License (California) required at hire.
- National Provider Identifier required at hire.
- Advanced Cardiac Life Support within 3 months of hire from American Heart Association.
- Basic Life Support within 3 months of hire from American Heart Association.
- Demonstrate knowledge of patient variables such as age, sex, and other physical characteristics/presentation and disease states as they relate to medication therapy.
- Comprehensive knowledge of acute care medications.
- Possess comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, and disease states, and the ability to relate this information to the patient's medical management plan.
- Demonstrated work experience performing aseptic technique and the ability to prepare sterile IV products and chemotherapeutic agents.
- Excellent verbal and written communication skills.
- Knowledge of Federal and State laws and regulations regarding the provision of pharmaceutical services.
- Passage of a standardized competency assessment or current incumbent in job code 60002, 60007, 67130, or 67150.
- Demonstrated ability to extract pertinent information from the patient or other sources and relate it to the patient's medical management plan.
- Experience operating pharmacy computer system(s) and other equipment.
- Comprehensive knowledge of emergency medications.
- Doctorate of Pharmacy, Pharm.D.
