**Job Summary**

Centers Plan for Healthy Living is seeking a skilled Speech Therapist to provide home-based speech therapy services to our clients. As a key member of our team, you will be responsible for evaluating patients referred for home care services, establishing a home care plan, and providing rehabilitative services for speech and language disorders.

**Responsibilities:**

* Conduct home visits to assess patients' speech and language abilities.

* Evaluate patients' conditions for swallowing or cognitive deficits.

* Develop and implement individualized treatment plans to meet established goals.

* Provide ongoing evaluation of patients' status and goals, modifying treatment programs as needed.

* Educate patients and their families on comprehensive and progressive home programs.

* Maintain accurate records of patients' progress, goals, and attendance.

* Select and use appropriate diagnostic and therapeutic techniques and materials.

* Document patients' responses to treatment on designated clinical progress notes.

**Qualifications:**

* Master's degree in Speech Therapy or a foreign degree equivalent as accepted by US Citizenship and Immigration Services.

* Valid New York State license as a Speech Language Pathologist.

* Excellent communication skills and the ability to interact well with diverse individuals.

* Basic computer skills.

* Ability to function independently with minimal supervision.

* Current driver's license, required insurance, and a car available for work.
