Position Summary:
The Clinical Nurse Specialist is a skilled professional responsible for assessing, managing, directing, and delivering nursing care throughout the patient's hospital experience. This role involves collaboration with other healthcare disciplines to ensure a patient-centered approach.
Key Responsibilities:
Essential Functions include:
- Conducting thorough nursing assessments and reassessments.
Criteria:
A) Performing age-appropriate admission or transfer assessments, involving family or guardians as needed.
B) Documenting findings accurately and comprehensively.
C) Assessing post-operative or post-procedure patients.
D) Evaluating and documenting educational and discharge needs for patients and families upon admission and throughout their stay.
E) Providing ongoing patient reassessments, documenting relevant observations according to the care plan and standards.
- Developing, coordinating, and evaluating a care plan based on assessment data, patient diagnosis, lab results, and physician orders.
Criteria:
A) Identifying short- and long-term goals based on patient needs.
B) Creating nursing interventions to achieve desired outcomes.
C) Integrating evidence-based practices into the nursing care plan.
- Delivering and documenting nursing interventions based on assessed patient needs and changes in condition.
Criteria:
A) Collaborating with healthcare team members to coordinate daily care plans.
B) Communicating patient care effectively, including accurate handoff reports.
C) Administering and documenting medications in accordance with policies.
D) Monitoring and documenting IV fluids and nutrition as per protocols.

H) Clarifying physician orders as necessary.
I) Implementing physician orders promptly and accurately.
J) Assisting physicians with procedures as needed.

M) Recognizing changes in patient conditions and taking appropriate actions.
N) Involving family members in care and decision-making when appropriate.
O) Identifying patient risks and taking necessary actions.
