We are excited to announce the opening for a Client Support Case Coordinator to join our experienced case management team. The ideal candidate will provide intensive case management services to clients receiving primary care at our facility. Responsibilities include coordinating cases with a multidisciplinary team to ensure adherence to treatment and evaluating necessary ancillary and support services. The successful candidate should be detail-oriented, possess excellent written and verbal communication skills, and be adept at managing multiple tasks simultaneously.

- Experience in delivering social support services to individuals living with HIV/AIDS.

Position Summary:
Under general supervision, as part of a multidisciplinary team, the Client Support Case Coordinator will work to improve patient retention in care for specialized populations. Responsibilities include ensuring eligibility for various medical and social services and assisting patients with necessary paperwork. This role may also involve providing assistance in accessing social, community, legal, financial, and other essential services, along with performing administrative functions to support efficient program operations.
Minimum Qualifications:
A high school diploma or GED along with 2 years of relevant experience, or a certification as a Certified Community Health Worker (CHW). Higher education degrees from accredited institutions may substitute for experience on a year-for-year basis. Verification of education and licensure will be required upon selection.
Key Responsibilities:
- Coordinate patient documentation to maintain eligibility for essential medical and social support services.
- Conduct patient interviews to complete intake applications and all necessary forms for medical and social support services.
- Collaborate closely with medical case managers to minimize barriers to obtaining required medical care and social services.
- Assist in identifying community resources that support medically and socially complex patients.
- Maintain updated information on commonly accessed community resources.
- Perform administrative tasks necessary for efficient program operations.
- Engage in professional development through completion of required departmental and New Mexico Department of Health competencies.
- Carry out miscellaneous job-related duties as assigned.
