We are seeking a skilled Nurse Practitioner or Physician Assistant to join our Home-Based Medical Care team in Brookhaven, NY. As a member of our interdisciplinary team, you will play a critical role in delivering high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.

About the Role

This is a unique opportunity to join a clinician-led care organization that is revolutionizing the way healthcare is delivered. As a Home-Based Medical Care Specialist, you will work closely with our interdisciplinary team to develop and implement personalized care plans that address the physical, mental, and social needs of our patients.

Responsibilities
  • Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
  • Collaborate with our interdisciplinary team, including physicians, nurses, and social workers, to deliver high-quality, patient-centered care.
  • Prescribe medications and treatments as needed to manage patients' conditions.
  • Provide education and support to patients and their families to promote self-management and independence.
  • Participate in quality improvement initiatives to ensure the highest standards of care are met.
Requirements
  • Completion of a Nurse Practitioner or Physician Assistant accredited program.
  • Current state RN license (if Nurse Practitioner) or NP or PA license.
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
What We Offer

We offer a competitive salary range of $88,000 to $173,200 annually, as well as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution.
