We are seeking a highly skilled and motivated Pharmacist to join our team at BrightSpring Health Services. As a Pharmacist, you will play a critical role in providing exceptional patient care and support to our clients in long-term care settings.
Key Responsibilities- Evaluate Medication Orders: Review physician medication orders for appropriateness, potential interactions, and route of administration, making recommendations for therapy changes as needed.
- Patient Database Management: Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Lab Test Ordering and Evaluation: Ensure that appropriate laboratory tests are ordered, evaluated, and discussed with the healthcare team to monitor high-risk conditions.
- Clinical and Technical Support: Collaborate with the healthcare team to provide clinical and technical support as needed, including documentation of all communication with physicians, nurses, and other team members.
- Medication Safety and Compliance: Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
- Error Reporting and Documentation: Inform the Pharmacy Manager of medication-related errors and maintain accurate documentation in accordance with federal and state regulatory agencies and policy.
- Education: Bachelor's Degree in Pharmacy; Pharm D preferred.
- Experience: One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
- Skills: Detail-oriented, goal-oriented, team player, excellent oral communication skills.
