Conduct comprehensive admission assessments to establish a clear understanding of each patient's condition.
Documentation:
Maintain meticulous records that reflect the patient's physical, mental, and emotional health, ensuring all care plans and interventions are accurately recorded. Regular audits of documentation from multiple patient charts are required.
Ongoing Evaluation:
Perform shift-to-shift reassessments to monitor patient progress and needs.
Care Transition:
Complete all necessary transfer and discharge documentation, assessing patient needs to determine the appropriate level of ongoing care.
Medication Management:
Adhere to the five rights of medication administration, clearly communicating with patients and their families about prescribed medications.
Patient Safety:
Evaluate patients for potential safety risks upon admission and implement necessary interventions to mitigate identified risks.
Education and Support:
Assess the educational needs of patients and their families, providing tailored instruction and evaluating their understanding of the information presented.
Leadership and Delegation:
Demonstrate effective leadership by coordinating nursing care delivery, supervising team members, and maintaining accountability for delegated tasks.
Skin Integrity:
Regularly assess and implement preventive measures for patients at risk of skin breakdown, following established protocols.
Compliance and Standards:
Uphold nursing standards of practice, ensuring adherence to unit-specific competencies and fulfilling all compliance responsibilities associated with the role.
Qualifications:
Education:
Associate's degree in nursing required.
Licensure:
Must possess a valid nursing license in the state of practice.
Experience:
Experience ranging from zero to six months is acceptable.
Certifications:
Basic Cardiac Life Support (CPR) certification is preferred within three months of hire.
Work Environment:
Physical Demands:
This role requires significant manual dexterity and the ability to exert considerable physical effort.
Occupational Hazards:
Exposure to various occupational risks is inherent in this position.
