Job Overview

A Home Health Aide plays a crucial role in delivering personalized care and support to clients in their homes, assisted living facilities, or healthcare settings. This position involves adhering to a structured care plan under the guidance of a registered nurse or therapist. The aide is responsible for observing and documenting patient conditions and care provided, ensuring accurate and timely records based on the care plan.

Work Schedule: PRN

Key Responsibilities

  • Assist clients with personal care tasks as outlined in the care plan, including hygiene, mobility, and elimination. Report any challenges in performing these tasks to the supervising nurse or clinical manager.
  • Maintain a safe environment for clients, utilizing equipment properly and ensuring safety protocols are followed.
  • Adhere to universal precautions during all interactions with clients.
  • Support clients and their families with household tasks as specified in the care plan, such as light cleaning and meal preparation.
  • Monitor clients' fluid and nutritional intake, vital signs, and general well-being, documenting any changes and reporting them as necessary.
  • Implement safe transfer and mobility techniques to assist clients in their daily activities.
  • Recognize when additional support from the healthcare team is needed and communicate this to the supervising nurse or clinical manager.
  • Respect the privacy and confidentiality of all clients and their families.
  • Document care provided and client responses accurately and submit reports promptly.
  • Operate within the scope of training and experience.
  • Ensure compliance with all relevant regulations and agency policies.
  • Manage a caseload effectively while delivering care in a cost-efficient manner.
  • Consistently meet or exceed the agency's service standards.
  • Conduct all professional activities with integrity and ethical standards.

Qualifications

Education Requirements:

  • High school diploma or equivalent.
  • Completion of a state-approved training program and certification as a home health aide, with proof of certification.
  • At least 75 hours of classroom and supervised practical training as a home health aide in accordance with regulatory standards.
  • Demonstrated competency through a passing score on a written assessment prior to independent patient care.

Experience Requirements:

  • Must be at least 18 years of age.
  • A minimum of 1 year of direct patient care experience in a healthcare or home care setting is required, with preference for community health or home health experience.

Licensure/Certification Requirements:

  • Valid driver's license and reliable, insured transportation.
  • Positive references from previous employers or professional contacts.
  • Successful completion of a criminal background check.
