A Home Health Aide plays a crucial role in delivering personal care and related services to patients in their homes, assisted living facilities, and rehabilitation centers. Responsibilities include observing and documenting patient conditions and care provided, ensuring timely and accurate records based on the patient's care plan.
Work Schedule: PRN
Key Responsibilities- Assist patients with personal care tasks as outlined in the care plan, including hygiene, mobility, and elimination. Report any challenges to the supervising RN or Clinical Manager promptly.
- Maintain a safe environment for patients, utilizing equipment correctly and safely (e.g., footstools, side rails, oxygen).
- Adhere to universal precautions in all patient interactions.
- Support patients and families with home maintenance tasks as specified in the care plan, such as light housekeeping and meal preparation.
- Monitor and document essential health metrics, including fluid and nutrition intake, temperature, pulse, respiration, and skin condition, reporting any significant changes.
- Implement safe transfer and ambulation techniques, ensuring proper positioning and range of motion.
- Identify ongoing needs for intervention from other members of the home health team and communicate these to the RN or Clinical Manager.
- Uphold the confidentiality and autonomy of all patients and their families.
- Document care provided and patient responses accurately and submit records in a timely manner.
- Perform duties within the scope of training and experience.
- Comply with all relevant local, state, and federal regulations, including Medicare guidelines and agency policies.
- Manage a caseload effectively while delivering care in a cost-efficient manner.
- Consistently meet or exceed the agency's service standards.
- Conduct all professional activities ethically and responsibly.

QualificationsEducation:
- High school diploma or equivalent.
- Completion of a state-approved training program and certification as a home health aide, with proof of certification.
- Minimum of 75 hours of classroom and supervised practical training as a home health aide in accordance with regulatory standards.
- Successful completion of a written competency test prior to independent patient care.
Experience:
- Must be at least 18 years of age.
- At least 1 year of direct patient care experience in a healthcare setting is required, with preference for community health or home health experience.
Licensure/Certification:
- Valid driver's license and reliable, insured transportation.
- Positive references from previous employers or professional colleagues.
- Successful completion of a criminal background check.
