Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Andwell Health Partners. As a Physical Therapist, you will provide high-quality physical therapy services to clients in their homes and communities.
Key Responsibilities- Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association.
- Develop, implement, and coordinate a plan of care that incorporates goals consistent with the client's age, condition, and needs.
- Implement client care plan activities based on treatment goals towards independent living for the client/family.
- Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers, and pay sources.
- Bachelors, Masters, and/or Doctorate Degree in Physical Therapy from an accredited institution.
- Current PT license in the State of Maine.
- Current CPR card required.
- 1-year Physical Therapy experience preferred. Home health experience preferred.
- A reliable car with insurance.
