Job Title: Orthopedic Nurse Coordinator

At Yale New Haven Health, we are seeking a highly skilled and compassionate Orthopedic Nurse Coordinator to join our team. As a key member of our clinical team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare providers.

Responsibilities:
  • Coordinate referrals and initial patient contact with clinical teams and attending physicians, identifying additions or deletions.
  • Maintain contact with patients, families, and referring physicians across the continuum of care, conveying pertinent medical information in collaboration with attending physicians and/or fellows.
  • Collaborate with management and social work to determine how best to address issues related to patient benefits and medical reimbursements.
  • Facilitate diagnostic, admitting, surgical, and/or treatment follow-up by communicating regularly in rounds with the provider team.
  • Collaborate with patient caregivers and other healthcare team members regarding current medical plans of care, attending interdisciplinary meetings as a representative of the service.
  • Maintain outpatient follow-up by assessing patient needs, concerns, and questions, providing education and support, and collaborating with the appropriate individuals to facilitate transitions and coordination of follow-up care.
  • Facilitate transitions of care by providing updated medical information, including reports of medications, procedures, and applicable details, and continuing care plans arranged.
  • Participate in enhancing the professional development of nursing staff and other healthcare providers involved in patient care, facilitating self-care of patients through patient and family education.
  • Function as an educational resource to staff nurses and other healthcare providers in the disease unit and/or quality initiatives.
  • Collaborate with physicians and nursing staff on the development of Patient Care Standards.
  • Collaborate with nurses in assessing the learning needs of patients and their families and providing education support when needed.
  • Provide written information to patients and their families pertaining to illness or surgical procedures and develop teaching tools as needed.
  • Convey information pertaining to hospital policy and procedure to resident staff, faculty, and other members of the service.
  • Serve as a primary contact for patients and families.
  • Facilitate a team approach to total patient care.
  • Perform ongoing assessment and management of patient care.
  • Coordinate care of multiple providers across the care continuum.
  • Assist in the coordination of the clinical program through interaction with internal and external contacts.
  • Participate in safety and quality improvement efforts.
  • Facilitate the integration of documentation and information systems.
  • Assist in marketing and community outreach efforts.
  • Assist in the coordination of the clinical program.
  • As a member of the patient care team, provide direct patient care during clinic visits in collaboration with the house staff and nursing staff.
  • Facilitate the continued care of patients following discharge through consultation with patients, families, and home care agencies, managing medical prescriptions and treatments to pharmacies or home care agencies under the supervision of attending physicians and/or fellows.
  • Remain current regarding issues pertaining to the medical specialty to enhance clinical practice.
  • Attend conferences as indicated.
  • Collaborate with research investigators and protocol personnel.
  • Foster academic and clinical investigation through participation in research projects in the specialty practice area and related fields of study, participating in literature review, data collection, and data entry.
Qualifications:

Bachelor's Degree in Nursing required.

3-5 years of experience with surgical patient population, inpatient or outpatient. Ortho and/or Spine experience preferred.

CT RN license.

Knowledge of physiology involving the disease unit. Excellent communication skills.

Minimum 1 year experience in orthopedic preferred. At least 2 years' experience in nursing, ambulatory experience preferred.
