Job Overview

We are currently seeking a Case Manager to serve as a vital link between clients and external resources.

Compensation: $18 - $20 per hour

Key Responsibilities:

  1. Act as the primary contact for families and external partners, addressing any issues that may arise during the treatment process.
  2. Provide education to families and stakeholders regarding treatment methodologies, philosophies, and the importance of parental involvement.
  3. Document and relay sensitive information in accordance with policies, ensuring professionalism and timeliness.
  4. Oversee the completion of essential documentation, including consents, releases, and treatment records, adhering to all relevant regulations.
  5. Track and report on case management activities.
  6. Verify financial and clinical eligibility for services prior to initiation, assisting families with financial arrangements and resource referrals.
  7. Ensure that financial eligibility information is communicated to the Accounting/Payroll technician and complete billing for services within required timelines.
  8. Lead and coordinate activities related to referrals, admissions, assessments, and discharges, in compliance with organizational policies.
  9. Take responsibility for the design and implementation of treatment plans, ensuring alignment with treatment objectives and progress monitoring.
  10. Facilitate collaboration among treatment teams and external entities to address any external factors affecting treatment.
  11. Coordinate meetings, intake sessions, and case conferences, ensuring thorough evaluations and timely revisions of treatment plans.
  12. Represent the agency in custody hearings and other legal proceedings when necessary.
  13. In specific service areas, may act as a medication representative, coordinating with psychiatrists for effective psychopharmacological treatment.
  14. Work with management to identify and cultivate new referral sources while maintaining existing relationships.
  15. Participate in training and professional development opportunities to stay informed about current trends in mental health services.
  16. Monitor daily service delivery and collaborate with other providers to ensure coordinated treatment efforts.
  17. Maintain accurate records and reports in compliance with legal and organizational standards.
  18. Support strategic goals and revenue cycle management in collaboration with all teams.
  19. Assist in achieving a balance between service efficacy and maximum occupancy.
  20. Guide parents towards educational resources and training to enhance their skills in child management and problem-solving.
  21. Discuss and coordinate transportation and medication management needs as appropriate.

Additional Responsibilities:

  1. Contribute to marketing and branding initiatives.
  2. Engage in special projects and serve on committees as assigned by management.

Qualifications:

  1. Bachelor's degree in a relevant social science field or equivalent experience.
  2. Preferred certification in Case Management.
  3. Valid driver's license required.
