We are seeking a skilled and compassionate Licensed Practical Nurse/Licensed Vocational Nurse to join our team of dedicated healthcare professionals. As a key member of our care team, you will provide high-quality patient care in a home-like setting, ensuring the dignity and independence of our clients.
Responsibilities- Provide one-on-one care to clients, following their individualized plan of care and maintaining their dignity and independence.
- Monitor clients' vital statistics and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
- Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
- Ensure infection control policies are maintained at all times, reporting any possible communicable diseases or infections and implementing infection control procedures.
- Accurately document nursing actions, communicate with family, case manager, and physicians, and document progress and outcomes for established goals.
- Order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
- Monitor incidents involving individuals, complete incident reports, and inform case managers of any incidents.
- Ensure all medical equipment is maintained and report any faulty equipment to the Durable Medical Equipment department for repair.
- Proof of current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- Current Cardiopulmonary resuscitation certification.
- Ability to work in a home-like setting.
- Ability to bend, stoop, push, pull, sit, and walk for periods of time.
- Able to utilize proper body mechanics while lifting 50 pounds.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Must have completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.

Salary and BenefitsUSD $35.00 per hour.
