Our team of experienced clinicians offers a range of services, including primary care, palliative care, and hospice care, to individuals in New York, New Jersey, Maryland, DC, Virginia, and Georgia.
Job SummaryWe are seeking a skilled Advanced Provider to join our team and provide high-quality care to homebound and palliative care patients. As a key member of our clinical team, you will work closely with our physicians to assess patient needs, develop personalized care plans, and deliver compassionate care to our patients.
Responsibilities- Provide primary care to patients in their homes, including assessing their health, creating care plans, and coordinating with our clinical team.
- Develop and implement treatment plans, including ordering diagnostic tests and prescribing medications as needed.
- Communicate effectively with patients, families, and caregivers to ensure seamless care coordination.
- Participate in quarterly medical staff meetings to stay up-to-date on best practices and share knowledge with colleagues.
- Board certification as a Nurse Practitioner or Physician Assistant in the State of New Jersey.
- Strong clinical skills and experience working with geriatric patients, preferably in a home care setting.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
- A flexible schedule that allows you to work at your own pace and see patients during hours that suit you.
- Autonomy to make decisions and take ownership of patient care, with support from our in-house clinical team.
