About the Role

As a Home Health Physical Therapist, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes. Your expertise and compassion will enable patients to regain independence, manage pain, and improve overall well-being.

Key Responsibilities
  • Evaluate patient needs and develop tailored treatment plans to enhance functional status, mobility, endurance, and quality of life.

  • Collaborate with healthcare teams, including physicians, nurses, and caregivers, to ensure seamless care coordination and communication.
  • Maintain accurate records and documentation, adhering to regulatory requirements and LHC Group standards.
Requirements

To be successful in this role, you should possess:

  • A current Physical Therapy licensure in the state of MA.
  • A strong background in home health rehabilitation, with experience working with diverse patient populations.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and interdisciplinary teams.
  • A commitment to ongoing education and professional development, staying abreast of industry advancements and best practices.
