Licensed Vocational Nurse Modesto

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team at Compassionate Care Home Health Agency in Modesto. As an LVN, you will play a vital role in providing high-quality patient care and contributing to the well-being of our patients and their families.
Job Summary:The Licensed Vocational Nurse will work under the supervision of a registered nurse to provide direct patient care, implement treatment plans, and collaborate with the healthcare team to achieve positive patient outcomes. Responsibilities:- Provide direct patient care as defined in the state Nurse Practice Act.
- Implement a plan of care initiated by the registered nurse.
- Provide accurate and timely documentation consistent with the plan of care.
- Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Use and/or prepare equipment and supplies effectively and efficiently adhering to aseptic technique.
- Participate in personal and professional growth and development.
- Perform other duties as assigned by the registered nurse.
- Graduate of an accredited practical nurse or vocational nursing program.
- One year of nursing experience; three years preferred. Community health/hospice or medical/surgical experience is preferred.
- Sensitivity to patient needs with the ability to express patience and compassion in dealing with them.
- Currently licensed as an LPN/LVN in California.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Strong commitment to providing the highest quality care.
- Possesses and maintains current CPR certification.
- Understand the philosophy of the hospice concept. Also understands the needs of the terminally ill.
