Job Title: Licensed Vocational Nurse Modesto

We are seeking a compassionate and skilled Licensed Vocational Nurse to join our team at Compassionate Care Home Health Agency in Modesto. As a Licensed Vocational Nurse, you will play a vital role in providing high-quality patient care and contributing to the well-being of our patients and their families.

Job Summary:

The Licensed Vocational Nurse will work under the supervision of a registered nurse to provide direct patient care, follow a comprehensive plan of care, and work collaboratively with the healthcare team to achieve positive patient outcomes.

Responsibilities:
  • Provide direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Participate in coordination of hospice services, appropriately reporting identified needs to the interdisciplinary group.
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique.
  • Provide appropriate pain/symptom management, evaluating patient response to treatments/medications.
  • Participate in personal and professional growth and development, as well as organization's quality assessment and performance improvement teams and activities.
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • One year of nursing experience; three years preferred, with community health/hospice or medical/surgical experience preferred.
  • Sensitivity to patient needs with the ability to express patience and compassion in dealing with them.
  • Currently licensed as an LPN/LVN in California.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
