As a home health physical therapist, you will have the opportunity to work with patients in their homes and communities, providing one-on-one care and empowering them to move freely and live fully.

Key Responsibilities:

  • Deliver high-quality physical therapy services to patients in their homes and communities
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Provide education and training to patients and families on physical therapy techniques and strategies

Requirements:

  • Licensed as a Physical Therapist by the Delaware state Board of Physical Therapy Examiners and/or part of the PT compact program
  • Graduate of an accredited university with a minimum degree of Bachelor's in Physical Therapy recognized by the APTA
  • Basic Life Support CPR Certification

Physical Requirements:

  • Must be able to lift up to 25 pounds at times
