Suncrest Hospice is seeking a highly skilled and experienced professional to fill the position of Lead Coordinator of Afterhours Services. As a key member of our team, you will play a critical role in ensuring the delivery of exceptional care and service to our patients and their families.
Key Responsibilities- Provide lead coordination leadership support for our afterhours/on call team, working Friday-Monday from Friday at 5pm to 8:30am Monday.
- Coordinate and manage all aspects of the schedule and staffing during the weekend and after-hours, as well as act as Administrator on Call.
- Collaborate with weekend Social Services Team to coordinate visit needs.
- Complete discharge audits and updating patient assignments list.
- Assist with processing, managing, and scheduling all after hours referrals.
- Delegate visits to field staff as needed.
- Schedule and manage weekend On Call Schedules.
- Function as the Administrator on Call.
- When necessary, visit patients as needed to provide back up for field staff.
- Assist with training and education of weekend afterhours employees.
- Keeps current with in-services and educational requirements.
- Registered nurse in the State with current licensure to practice professional nursing in the State. Management Experience preferred.
- Bachelor's degree in Nursing from an accredited program by the National League for Nursing preferred.
- Hospice experience required (5 or more years) Required.
- Experience in a supervisory and/or management role leading clinical and support teams.
- Strong communication and customer service skills.
- Must be a licensed driver with an automobile that is insured in accordance to state and/or Organization requirements and is in good working order.
- Has excellent observation, delegation, verbal, and written communication skills.
- Knowledge of business management, governmental regulations, and CHAP standards.
