Overview:
The Quality Improvement Coordinator serves as a pivotal Registered Nurse dedicated to ensuring precise evaluations, effective care strategies, and optimal patient outcomes within the home health sector. This role requires a profound understanding of home health regulations, continuous education, and a commitment to enhancing care quality.

Key Responsibilities:
  • Preferred certification in coding.
  • A minimum of three years of experience in home health services.
  • In-depth knowledge of federal and state regulations relevant to home health operations.
  • Expertise in Quality Assurance Performance Improvement (QAPI) methodologies, process enhancement, and outcome management.

The coordinator is accountable for delivering accurate, thorough assessments and ensuring the development of effective care plans by nursing staff and therapists.


Additional Duties:
  • Ensures correct and informed coding in accordance with established guidelines as validated by the assessing clinician.
  • Facilitates, coordinates, and monitors Quality Assessment Performance Improvement initiatives, focusing on outcomes management, including QAPI, adverse events, staffing efficiency, patient and employee satisfaction, and infection control.
  • Aggregates and reports compliance metrics and anomalies, assisting with inquiries from legal departments, insurance providers, and intermediaries.
